What is Selinexor 20 mg (Selinex) used for?

Seilnexor 20 mg (Selinex) is a prescription medication used to treat the following conditions in adult patients.

  • In combination with bortezomib and dexamethasone to treat multiple myeloma in adult patients who have received at least one prior therapy.
  • In combination with dexamethasone to treat multiple myeloma that has come back or did not respond to previous treatment.
  • Who has received at least 4 prior therapies? 
  • And whose disease did not respond to at least 2 proteasome inhibitor medicines, at least 2 immunomodulatory agents, and an anti-CD38 monoclonal antibody medicine.

On December 18, 2020, the Food and Drug Administration approved selinexor in combination with bortezomib and dexamethasone for the treatment of adult patients with multiple myeloma who have received at least one prior therapy.

It is not known yet if SELINEX is safe and effective in children less than 18 years of age. 

You have to undergo a necessary blood test before taking Selinex 20 mg, and then the first 3 months during treatment, and then as needed.

How should I take Selinex 20 mg?

  • Take the medication exactly as prescribed by your healthcare provider. 
  • If you have multiple myeloma, your healthcare provider will give you dexamethasone in combination with Selinex. Take dexamethasone exactly as prescribed.
  • Your healthcare provider will tell you how much to take and when to take it. Do not change or stop the dosage without your healthcare provider’s permission. 
  • Swallow the Selinex tablet whole with water; do not break, crush, chew, or divide the tablet. 
  • It is important to take enough fluids and calories to prevent dehydration and weight loss during the treatment. 
  • If you miss a dose of Selinex, take the next dose at the regular schedule time. 
  • If you take too much Selinex, call your healthcare provider right away. 

What should I avoid while taking Selinex 20 mg (Selinexor)?

If you have neurological side effects, do not drive or operate any heavy machinery until your neurological side effects go away. And avoid falling due to neurological side effects. 

Selinex 20 mg can cause serious side effects, including

  • Lower platelet count is a common side effect of this treatment, but sometimes it can be serious and cause death. To reduce the risk of bleeding, doctors may suggest lower platelet treatment medication. 
  • Low white blood cell count is also a common effect, but sometimes it can be severe. You may have a risk of increasing bacterial infection during treatment. Doctors may suggest antibiotics to reduce the risk of infection.
  • Sometimes serious infections, including upper or lower respiratory tract infections, such as pneumonia, and an infection throughout your body (sepsis) may happen that can even cause death. Inform your healthcare provider if any of the symptoms are seen. 
  • Neurologic side effects such as dizziness, fainting, decreased alertness, and changes in your mental status, including problems with thinking, seeing, or hearing things that are not really there (hallucinations). These are not that serious, but sometimes they can be life-threatening, so tell your healthcare provider if any of them are seen. 
  • Sometimes nausea, vomiting, and diarrhea can be severe and may increase the risk of dehydration. To reduce the risk, doctors may suggest anti-dehydration medication. 
  • Decreased sodium level in your blood.
  • Cloudiness and loss of transparency in the eyes are common, but sometimes can be severe.

Common side effects of Selinex include

  • Tiredness
  • Low red blood cell count
  • Constipation
  • Shortness of breath
  • Increased blood sugar
  • Changes in body salt and mineral levels in your blood
  • Changes in kidney and liver function.

Before taking Selinex 20 mg (Selinexor), tell your healthcare provider all the medical conditions you have, including

  • If you have or had a recent active infection
  • If you are pregnant or plan to become pregnant, Selinex can harm your unborn baby. 
  • If you have a history of bleeding problems
  • If you are taking prescription, non-prescription, vitamin, and herbal medications.

Talk to your healthcare provider about infertility due to using Selinex.

Selinex can cause fertility problems in both men and women. If you are planning to take the medication, consult with your healthcare provider before taking it and ask about the risk of infertility. They may suggest necessary medications to prevent infertility problems.

Use effective birth control during the treatment.t

Both males and females need to use effective birth control during the treatment. Continue this process at least one week after the last dose.  Tell your healthcare provider right away if you become pregnant during the treatment. Hold breastfeeding for at least one week after the last dose, as it may pass through the breast milk.

Store Selinex at 86°F (30°C). It comes in a child-resistant blister pack. Keep and all medicines out of the reach of children.
